Time Sheets
Time Sheets
- A time sheet is a record of hours worked in a 2 week pay period with the beginning and ending dates for that time frame.
- The hours on the time sheet must be accurate and correspond with the hours on your work schedule (unless otherwise approved by the Nurse Supervisor).
- An incomplete or improperly completed time sheet may result in a delay in receiving a pay check.
Time Sheets must include:
- Attendant Care Worker's Name
- Employee Identification Number (EIN)
- Hours Worked
- Attendant Care Worker Signature
- Client's or signature of person authorized to sign for client
- Client's Name Printed
- A separate time sheet must be submitted for each client.
Time Sheets must be submitted to payroll at Herbert K Abrams Public Health Center

no later than the second Wednesday of the pay period by 4:30
p.m. Pay Period Calendar
Time sheets must be submitted no more than 2 weeks before or
2 weeks after the pay period.
Attendant Care Workers will not be paid for the following:
- Hours worked but not authorized in advance by the Case Manager or approved by the Nurse Supervisor.
- Vacation time or other time off such as personal illness.
- When the client is in the hospital, nursing home, or is not home.
- If the client is out of Pima County area.
- Call to Jury Duty.
Falsification of the time sheet is cause for termination and will be referred for criminal prosecution
Instructions for Completion of Time Sheet:
A) Enter ACW Name
B) Enter Employee Identification Number (EIN)
C) Enter date Pay Period ends
D) Enter total hours worked for each day.
E) Total the number of hours worked for week one of pay period.
F) Total the number of hours worked for week two of pay period.
G) Employee (ACW) Signature.
H) Client's Signature
I) Client's Printed Name.
J) Goldenrod (dark yellow) copy is for client.
K) Pink copy for Attendant Care Worker.
L) Submit to payroll the white and yellow copies.
