Disease Management
Division
The Disease Management
Division is an integral component of the PHS organizational
philosophy and service delivery system. The goals of the Disease Management
Program are to promote wellness, prevent adverse outcomes and to identify
members who have a problem or condition who would benefit from a preventive
health/disease management program that offers case management, education and
insight into an alternative lifestyle. The Pima Health System commitment to each
member is to promote prevention, wellness and maintenance of optimal health by
providing and utilizing case management activities, member education, assistance
with obtaining services and treatment.
Covered services include:
- Pre-disease screening for
chronic conditions, including but not limited to cervical cancer screening,
mammograms for breast cancer screening, prostate and colon examinations,
screening for high blood pressure, and adult immunizations.
- Educational programs that
target the reduction of established risk factors.
- Educational programs that
encourage good health behaviors and self-care management.
- Case Management activities
for members that are identified as needing one on one assistance.
- Organ/Tissue Transplantation
coordination.
- Health Assessment/Health
Profile completion with Case Management if indicated.
The Disease Management
Division Manager and the Acute Care Case Manager(s) are dedicated to this service
component.
