The Community
Services System is a home and community based services program for
frail elderly and younger disabled adults. It is a program of Pima
Health System in partnership with Pima Council on
Aging.
MISSION
The Mission of the Community Service System (CSS) is to assist frail
elderly and younger physically disabled residents of Pima County in
remaining safely at home for as long as possible and enhancing each
individual’s quality of life and ability to remain independent.
THE
COMMUNITY SERVICES SYSTEM
- Was established
in 1978 by Pima County Government and Pima Council on Aging
- Is a program of
Pima Health System
- Provides
non-medical home and community based services
- Assists client to
remain independent and in their own homes
- Provides a case
manager to assist each client
- Offers caregiver
training and support services to informal caregivers
WHO IS ELIGIBLE
Pima County residents with the greatest social and economic need who
are:
- Frail elderly (60
and older); or
- Physically
disabled (18 – 59); and
- Require
assistance with self-care

will evaluate your need for assistance
If you qualify a
case manager will call to arrange an in-home assessment visit
If you are
eligible, you and your case manager will develop an individual
care plan
The case manager
will authorize, arrange for and monitor services

The Community
Services System, through the supportive services it provides, helps
Pima County residents to continue to live safely in their homes for
as long as possible.
Donations are
accepted and appreciated and help to provide services to more people
in need. Respite services require the client to pay a share of the
cost. No one is denied services based on their ability to pay.
