PHS has three distinct pharmacy benefit plans. To proceed, you must choose one of the three plans below.

• The Ambulatory (AMB) Plan (sometimes referred to as the Acute plan) represents the vast majority of our members and the AMB Formulary is the core formulary for all three plans. 
  
• Arizona Long Term Care Service (ALTCS) members are split into two groups: Home and Community Based Services (HCBS) and Institutional (INST). 
  
• HCBS - members who live in their own homes or in a community assisted-living environment. 
  
• INST - those members who reside in skilled nursing facilities. If you are not sure which group your patient is in, you can go to the Provider Assistance section of this website and find the information under the on-line PCP Panel Inquiry. Panel members will be listed as either Acute or ALTCS.

You may print each formulary in part or in its entirety. The Sort By Therapeutic Class Option is the easiest way to print off each formulary or you can sort by generic or brand name and select "ALL" and print the whole formulary for that plan by alpha.
